The Champlain Regional Stroke Network (CRSN) works closely with its many partners in the region and province to ensure quality stroke care and improve the outcomes and experience of persons with lived experience of stroke and caregivers. Various events and activities help us reach these goals. Healthcare providers and members of the public are welcome to request that an event or resource be shared on the CRSN website.

Requests will be considered for sharing based on these guidelines.  After reviewing the guidelines you may submit your request using the form link below. 

Approval decisions on submissions received by the 15th of each month will be communicated by the 15th of the following month. Submissions from current partners such as Heart and Stroke Foundation, March of Dimes, and Ontario Caregiver Organization may be processed more quickly.

Community Stroke Rehabilitation is defined as care provided by an interprofessional team with stroke expertise, guided by best practice guidelines, in the setting that best meets the needs of the person with stroke (outpatient clinic and home settings).


If your patient would benefit from Community Stroke Rehabilitation, please have the primary care provider or most responsible physician complete the referral form and fax to 613-745-8243. Eligibility and exclusion criteria can be found on page 3 of the referral.
